From advance care planning and end-of-life doula services to dementia therapy and carer wellbeing support, we are here to empower, educate, and advocate for those who need it most.

With over 18 years experience, Kate is here to offer a professional and passionate approach, specialising in older adults, dementia care and safeguarding concerns, championing independence.

Lauren
Associate & Independent Social Worker
With over 10 years experience within health and social care, Lauren has a wealth of knowledge and expert skills in assessments, safeguarding, mental capacity, continuing healthcare, dementia support, trauma and hospital discharges.

Assessments & Reports
• Care Act Needs Assessment – £250
Holistic, strengths-based assessment with report and recommendations.
• Mental Capacity Assessment – £300
Full MCA assessment and written report for decision-specific capacity.
• Best Interests Decision Support – £200
Facilitation of best interest discussions, documentation and summary.
• Lasting Power of Attorney application for Health & Welfare or Finances £200
Support with completion of application to gain Lasting Power of Attorney through Office of Public Guardian.
• Mental Capacity Assessment + Court of Protection Report for Health & Welfare or Finances £400
Full MCA assessment and report completed as required by Court of Protection to gain deputyship.
Advocacy & Case Support
• Health & Social Care Advocacy – £100/hour
Support with Continuing Healthcare, complaints, appeals, or complex care navigation.
• Care Planning & Coordination Package – £450 per week (up to 5 hours)
Liaison with services, family and professionals to coordinate care.
Self-Neglect & Hoarding Support
• Self-Neglect Intervention (6-week programme) – £600
Practical and therapeutic support to build trust and address risks.
• Hoarding Support Programme (6-week block) – £750
Compassionate, relationship-based approach to reduce harm and improve wellbeing.
Hospital Discharge & Transitions
• Discharge Coordination Support – £300 per day
Helping individuals and families plan safe, person-centred discharges.
• Independent Readiness Review – £250
Objective review of discharge plans and identified barriers.
Quality & Improvement
• Case File Audit / Thematic Audit – £350 per audit
Reviewing quality and compliance against Care Act, MCA and safeguarding standards.
• Practice Improvement Report – £750
Independent review with findings, analysis, and recommendations.
Supervision, Reflective Practice & Peer Support
• 1:1 Reflective Supervision – £90 per hour
For social workers, team leads, or wellbeing professionals.
• Group Supervision (up to 3 people) – £180 per session
Add £30 per additional participant.
Training & Workshops
• Professional Training (2-hour workshop) – £400
Topics include safeguarding, MCA, self-neglect, strength-based practice.
• Family / Carer Workshops – £300
Supporting decision-making, navigating care systems, understanding CHC.
Practice Education & Student Placements
• Practice Educator Supervision – £120 per session
• Placement Support Block (4 weeks) – £1,000
Organisational Consultancy
• Process Review & Improvement Strategy – £1,200 per project
Reviewing systems such as discharge, safeguarding, or social work processes.
• Interim/Agency Social Work Support – £350 per day
Flexible support for providers, local authorities, or projects.
Community & Preventative Support
• Therapeutic Animal-Assisted or Nature-Based Sessions – £150 per hour
Delivered jointly with Fields Farm CIC to promote wellbeing and recovery.
• Green Prescription Pathways – price on request (joint delivery model)

Friendly, non-medical support for those who are isolated or in need of company.
Dementia iCST Therapy £20 per participant per session
Cognitive stimulation therapy to help maintain memory, communication, and wellbeing.
End-of-Life Doula Services £25 per hour - delivered by Charlotte.
Emotional and practical support for individuals nearing end of life and their families.
